Add Hidden Fields to /Report/ page #149

Original report by Kye Hoover (Bitbucket: eykrevooh, GitHub: eykrevooh).

Add a hidden field for each drop down option. When the a select is done the hidden fields value is updated.

Remove JS the enables all disabled elements on submit. No longer needed if there are hidden fields.
