When a user right-clicks a layer, a popup appears to allow some customization. A link to the "submit a request" form could be added, along with pre-filled layer identifiers (name, ID, row #). Consider if it would be possible to make a link in the spreadsheet that takes you right to the row # in question -- possibly need to use a formula in the form submissions sheet to build the link?
