How do we refer to the MIDS levels? All caps, all lower case? #116

Currently, we are using mids0, mids1 and so forth. This is not consistent with the labels we use for Information Elements and we do not consistently refer to the levels in the same manner across documentation.

Suggestion to use MIDS0, MIDS1 and so forth from now on?

I prefer all caps, I think it stands out more and I like the consistency

MIDS is an acronym. So, it should be capitalized to follow the common practice. I also prefer it as well.
