Templatize document for initial eval of component proposals

We need a template document for doing our initial evaluation ("first pass" UX and accessibility evaluation) of component and pattern proposals. We can just make the glossary component proposal eval doc into a template.

Created a draft of the component proposal initial evaluation template. I highlighted most of the things we need to edit in yellow to hopefully make them easier to spot. I also left some of the content from the glossary proposal eval in case having actual examples is helpful.
