Create and distribute a survey to government teams to assess their needs around accessibility.

The purpose of the survey is to establish a baseline of awareness of the existing accessibility resources (such as the testing checklists) already provided by USWDS, and their usage and utilization.

Additionally we hope to establish what else the community needs regarding accessibility and how a11y can be a value add for the community when deciding to implement our components.

The a11y team and UX research team met today to discuss goals of the survey - see notes.

We are all in the process of getting access to Qualtrics. Amy C already sent the request to OCE and we're awaiting next steps.
